After you are signed up by your pulmonologist, we will contact you for an intake interview at the Diaphragm Center.<\/p>\n

If needed, you can reach the Diaphragm Center at 0031 623913302. Please clearly state your name, date of birth and phone number on our message service.<\/p>\n

For urgent medical attention, contact your general practitioner.<\/p>\n

Appointment Change<\/h5>\n

Would you like to change your intake appointment? If so, you will find the contact information in the email you received about this.<\/p>\n

Once you are scheduled for surgery, contact is through the hospital where the surgery will take place.<\/p>\n
